Declining participation, withdrawal, opting out or objections

You can decline to participate in the survey by using the consent form included in your survey pack or you can contact IQVIA using their free Helpline number: 0800 7831775.

If you agreed to participate in the survey, you may be able to withdraw from the survey up to the point when the data is ready for analysis. If you would like more information about how to withdraw, please contact IQVIA by calling their Helpline 0800 7831775.

If you do not want to be included in any part of the HALO study, then to opt out of survey and/or data linkage component, please contact:

If you want to complain about how researchers have handled your information, you should contact the research team. If you are not happy after that, you can contact the University of Leeds Data Protection Officer via email: dpo@leeds.ac.uk.

If you are not happy with their response or believe they are processing your data in a way that is not right or lawful, you can complain to the Information Commissioner’s Office (ICO) (www.ico.org.uk or 0303 123 1113).

This will not affect any of the care you receive.
